Part-time, up to 29 hours per week
Pay:
$17.50- $18.75

Responsibilities
The
Community Services Direct Support Professional
is responsible for providing direct care through implementation of Habilitation Plans. Specific duties and responsibilities include, but are not limited to:
Provide supervision and all direct care required to maintain safety and comfort of the person receiving services
Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and exploration
Development and planning of inclusive community activities
Establish and maintain positive relationships with area businesses
Assist people receiving services with completing daily or routine domestic or personal care tasks
Foster and promote skills leading to independent living
Assist with the development of Habilitation Plans and following these plans for training and assistance
Provide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required
Provide supervision to people supported during program hours and during community outings as to ensure their health and safety
Complete required documentation in conformance with rules, regulations, policies and procedures, and billing practices
Requirements
High School Diploma/GED
Preferred: 2 years of experience working with the individuals with disabilities Or A combination of education and experience that in the opinion of the reviewing agency, exceeds the above standards of the program
Possess a valid driver’s license acceptable to the agency’s insurance carrier.

Salary : $18 - $19
