Position Summary

The Occupational Therapist provides comprehensive evaluations and individualized treatment for patients in both outpatient and home health settings. This position focuses on helping individuals improve independence, function, safety, and quality of life while delivering care that aligns with Achieve Therapy's mission and values.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop individualized treatment plans that reflect each patient's goals and priorities.
• Treat orthopedic, neurological, upper extremity, balance, cognitive, and functional conditions.
• Provide home health occupational therapy services within the local community.
• Educate patients and caregivers to promote confidence and long-term success.
• Collaborate with physicians, therapists, and healthcare providers to ensure exceptional continuity of care.
• Maintain timely and accurate documentation.
• Participate in team meetings, education opportunities, and quality improvement initiatives.
• Foster meaningful patient relationships built on trust, empathy, and encouragement.

QualificationsRequired

• Graduate of an accredited Occupational Therapy program
• Current Occupational Therapist license (or license eligible) in Minnesota and/or North Dakota
• CPR certification
• Valid driver's license and reliable transportation for home health visits

Preferred

• Experience in outpatient orthopedics and/or home health
• Excellent communication and interpersonal skills
• Strong clinical reasoning and problem-solving abilities
• Commitment to lifelong learning and evidence-based practice
