What You'll Do

• Provide physical therapy treatments under the direction of a licensed Physical Therapist

• Help residents improve mobility, strength, balance, endurance, and overall function

• Carry out individualized plans of care established by the PT

• Monitor resident progress and communicate changes to the supervising therapist

• Complete accurate and timely documentation using Net Health

• Collaborate with therapy, nursing, and other members of the interdisciplinary team

• Educate and encourage residents throughout their rehabilitation journey

• Maintain compliance with facility, state, federal, and professional standards

What We're Looking For

• Graduate of an accredited Physical Therapist Assistant program

• Current New York PTA license or eligibility for NY licensure

• SNF/LTC experience preferred

• Strong communication and teamwork skills

• Dependable, motivated, and ready to jump in when needed

• A positive attitude and passion for helping residents reach their goals
