Position: Direct Support Professional Day Habilitation Trainer

Day Habilitation Services: The Day Habilitation Program is about promoting independence and support for individuals with moderate to severe disabilities in the Human Services field. This program is available to people living in our 3 county coverage area. In addition to the activities that take place inside our facilities, you’ll also find us:
- Volunteering for local charities and churches
- Enjoying our local parks
- Shopping
- Eating at local restaurants
These are a few of the things a Direct Support Professional Day Habilitation Trainer will do:
- Implement goal plans and data collection to monitor progress.
- Transport individuals to volunteer and recreational activities in the Community. Also provide the necessary supervision, training and support during these outings.
- Maintain accurate daily participant records, charts, logs, and case/progress notes per the instruction of the Direct Supervisor and the individual treatment plan methodologies.
- Maintain an orderly, clean, safe, environment at all times.
- Order supplies for with approval from the Direct Supervisor.
- Assist an individual with toileting, if needed.
- Complete monthly progress notes, semi-annual and annual reviews as assigned by or in the absence of the Direct Supervisor.
- Assist in boarding vehicles and disembarking from vehicles.
- Monitor passengers and maintain orderliness on vehicle to ensure passenger safety and adherence to safety regulations.
- Assist in planning activities which will foster community integration.
These are some Special Requirements of a Direct Support Professional Day Habilitation Trainer:
- Candidate must be able to lift 50 pounds and not have any lifting restrictions which would prohibit him/her from assisting with toileting, transfers, physical intervention, lifting, etc.
- A valid driver’s license and good driving record acceptable to the Agency’s insurance carrier.
- Obtaining and maintaining certification in Medication Administration, CPR, First Aid and SCIP (this training and certification is provided by ACHIEVE.)
These are the Qualifications needed for a Direct Support Professional Day Habilitation Trainer. Prior experience is not necessary:
High School Graduate or GED with some knowledge of or experience working with developmentally disabled preferred.

A combination of education and experience that in the opinion of the reviewing agency meets or exceeds the above standards of the program
