Key Responsibilities
Clinical Oversight & Coordination
- Provide remote clinical supervision and direction to interdisciplinary hospice teams and office staff.
- Ensure plans of care are accurate, compliant, and aligned with physician orders and hospice regulations.
- Coordinate patient referrals, admissions, recertifications, physician orders, and staffing assignments.
- Facilitate admission reviews and case management oversight for assigned patient populations.
- Ensure assessment visits are completed within required time frames.
- Supervise, coach, and support clinical staff to ensure quality and continuity of care.
- Provide guidance on patient care procedures, documentation standards, and hospice eligibility criteria.
- Conduct regular staff meetings, performance evaluations, and ongoing clinical support.
- Assist with staff orientation, training, and ongoing education to meet regulatory and agency requirements.
- Promote compliance with state licensure, Medicare certification, and third-party payer requirements.
- Maintain current knowledge of federal, state, and local hospice regulations.
- Support audits, surveys, and performance improvement initiatives.
- Ensure accurate and timely documentation, orders, and records processing.
- Participate in quality improvement activities, including documentation review and data collection.
- Identify opportunities to improve clinical workflows, operational efficiency, and patient outcomes.
- Assist with medical community education and interdisciplinary collaboration.
- Provide leadership to clinical and administrative staff.
- Promote the agency’s mission, philosophy, and values.
- Support professional growth through education, mentorship, and performance feedback.
- Collaborate with leadership to ensure appropriate staffing and resource utilization.

Qualifications
- A heart to serve patients and families and a passion for providing the best possible care
- Education: Graduate of an accredited nursing school (BSN preferred). Current state license as a registered nurse.
- Experience: 2-3 years of supervisory experience as a registered nurse in an appropriate clinical care setting, home health, or hospice environment (preferred).
- Skills: Strong clinical knowledge of hospice nursing standards and practices. Excellent organizational, communication, and leadership skills. Ability to manage multiple priorities in a remote environment. Strong documentation, compliance, and workflow management skills.
- Required: Reliable transportation, valid driver’s license, and proof of auto insurance. Meets all agency health screening and testing requirements.
- Pay is determined by years of experience and location.
