Overview
We are seeking a compassionate and dedicated Home Health Care Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide in-home medical care to pediatric through adult patients. Night and Day shifts available in Northern Grand Rapids (Plainfield area), Sand Lake, Howard City, Big Rapids, White Cloud (Croton/Hardy area). This role involves delivering personalized nursing services in a home setting, ensuring patient comfort, safety, and well-being. The ideal candidate will have experience caring for patients with complex medical needs, including ventilator management, tracheostomy care, and feeding tube management.
Duties
- Deliver comprehensive nursing care to pediatric and adult patients in their homes, tailored to individual health needs.
- Able to manage and monitor ventilator support patients, ensuring proper operation and patient safety.
- Able to provide tracheostomy care, including cleaning, suctioning, and maintaining airway patency.
- Able to administer feeding through feeding tubes, ensuring proper nutrition and preventing complications.
- Assess patient conditions regularly and document clinical observations accurately.
- Collaborate with family members and caregivers to educate them on patient care routines and safety protocols.
- Follow all healthcare regulations and safety standards to maintain a safe environment for patients.
- Adjust care plans as needed based on patient progress and physician instructions.
Requirements
- Valid LPN or RN license in the state of practice.
- Experience caring for pediatric to adult patients with complex medical needs such as ventilator dependence, tracheostomy management, and feeding tube care.
- Knowledge of home health nursing practices, including infection control and emergency procedures.
- Ability to work independently with strong critical thinking skills.
- Compassionate attitude with excellent communication skills.
- Reliable transportation and willingness to travel to patient homes as required.

Pay: $33.00 - $37.00 per hour
Expected hours: 16.0 – 40.0 per week
