What are the responsibilities and job description for the RN for Case management in Adult Family Living program position at Accurate Care, LLC?
As a Registered Nurse for Accurate Care Adult Family Living, your main responsibility will be to conduct home visits for clients in the community. Your caseload will consist of clients who require monthly or bimonthly home visits. The visits will be scheduled by you when it works best for you. The flexability of this position allows you to schedule your clients according to what works best for you. Whether you are looking for full-time or part-time hours, we can accommodate your preferences. There is no overtime required, and we provide paid gas mileage for your travel between client visits.
THIS POSITION IS FEE FOR SERVICE
Job Requirements:
Must be a Registered Nurse in the state of Connecticut.
Must have AFL (adult family living) or AFC (adult foster care) experiance
Valid driver's license, car, and cell phone.
Strong organizational and time management skills.
Ability to work independently and manage a caseload.
Excellent communication and interpersonal skills.
Experience in home care or community nursing.
