Role Description
The MDS Coordinator-LPN is a full-time, at our Accura Healthcare of Newton. Responsible for overseeing the Resident Assessment Instrument (RAI) process and ensuring accurate and timely completion of Minimum Data Set (MDS) assessments. The MDS Coordinator collaborates closely with nursing, therapy, and social services to gather assessment data, verify documentation, and support reimbursement accuracy.
Daily responsibilities also include monitoring quality indicators, participating in care conferences, educating team members on MDS and care plan requirements, and contributing to continuous improvement in resident outcomes.

Qualifications
- Active state of Iowa Licensed Practical Nurse (LPN) license, or an active Nurse Compact License
- Demonstrated expertise with MDS assessments, regulatory requirements, and documentation standards
- Strong assessment, critical thinking, and clinical judgment skills, with attention to detail and accuracy
- Effective communication and collaboration skills to work with interdisciplinary teams and engage residents and families
- Organizational and time management abilities to handle multiple assessments, deadlines, and regulatory priorities
