Clinical Manager at a Home Health Agency

Clinical Manager position at a leading Home Health Agency. Our client is seeking a dedicated healthcare professional to lead a multi-disciplinary team and ensure the delivery of high-quality patient care. This role offers a unique chance to make a significant impact on patient outcomes and satisfaction while ensuring compliance with regulatory standards and company policies.

Key Responsibilities:

  • Leadership and Supervision: Lead and supervise a diverse team to ensure consistent, high-quality patient care.
  • Patient Visits: Conduct patient visits as needed to maintain oversight of care quality and satisfaction.
  • Regulatory Knowledge: Maintain comprehensive knowledge of federal, state, local, and Medicare regulations relevant to home healthcare.
  • Admissions and Care Plans: Oversee new admissions, execute physician orders, and develop comprehensive care plans for patients.
  • Compliance and Readiness: Ensure agency readiness for surveys and compliance with regulatory standards.
  • Staff Development: Assess staff needs, establish goals to enhance clinical services, and expand referral bases.
  • Productivity and Management: Monitor re-hospitalization cases, oversee productivity, and manage territory assignments.
  • Support and Coordination: Provide support to clinicians, address coordination issues, and ensure protocol adherence.
  • Relationship Building: Cultivate relationships with case managers and collaborate with the marketing team on referral strategies.
  • Training and Compliance: Coordinate training for new hires, manage disciplinary actions, and handle payroll adjustments.
  • Collaboration: Work with HR and other departments to address staffing needs and implement training programs.
  • Performance Evaluations: Conduct regular performance evaluations and provide constructive feedback to staff.
  • Strategy Implementation: Develop and implement strategies to improve patient outcomes and satisfaction.
  • Industry Engagement: Engage with external stakeholders to enhance patient care services and stay abreast of industry trends.

Team Management:

  • Recruitment and Hiring: Collaborate with Talent Acquisition for recruitment and interviewing of clinical staff.
  • Staff Development: Hire, train, and, when necessary, discipline staff to ensure a professional and competent workforce.

Compensation and Benefits:

  • Full Time
  • Competitive Pay: up to $195,000.00 per year

Requirements:

  • Minimum of 1 year Home Health experience
  • Minimum of 1 year management experience
  • 2 - 3 years of RN Nursing/Managerial Experience (Preferred)
  • Experience in Patient Care Coordination and Escalations
  • Clinical Coordination Managerial Experience
  • Re-hospitalization Reduction Expertise
  • Resource and Staff Management
  • Disciplinary and Performance Management
  • Regulatory Compliance and Risk Management
  • Valid RN license in California
  • Strong organizational, communication, and leadership skills
