We are hiring for full-time and per-diem roles across Indian River, St. Lucie, and Martin Counties.

Position Summary
The Physical Therapist provides skilled, patient-centered therapy services for both acute-level (Hospital at Home) and Medicare home health patients, focusing on mobility, fall prevention, functional independence, and successful recovery outcomes.
Key Responsibilities
- Perform comprehensive PT evaluations, including:
- Strength, balance, endurance, and functional mobility
- Gait analysis and fall risk assessment
- Transfers, bed mobility, and stair negotiation
- Activity tolerance and cardiopulmonary response
- Develop and implement individualized, evidence-based treatment plans
- Address functional decline related to acute illness, injury, or recent hospitalization
- Provide gait training, balance interventions, and mobility progression
- Educate patients and caregivers on:
- Safe mobility and transfer techniques
- Fall prevention and home safety
- Use of assistive devices and equipment
- Monitor patient progress and communicate changes with the interdisciplinary team
- Complete accurate and timely documentation, including OASIS assessments (preferred)
- Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce readmissions
- Active Florida Physical Therapist (PT) license
- Degree from an accredited Physical Therapy program (DPT preferred)
- Current CPR certification
- Valid driver’s license and reliable transportation
- Strong clinical reasoning, communication, and time-management skills
- Medicare home health experience
- OASIS experience preferred
- Acute care or hospital-based physical therapy
- Hospital at Home, home health, or transitional care experience
- Experience treating medically complex patients
- Familiarity with EMR systems
