What You'll Do- Conduct inhome assessments and create individualized care plans
- Provide clinical oversight, coaching, and support to caregivers and field nurses
- Ensure compliance with LHCSA and NYS DOH standards through accurate, timely documentation
- Collaborate closely with families, providers, and interdisciplinary team members
- Support staff education and quality improvement initiatives
- Active, unrestricted RN license in New York (BSN preferred)
- Minimum 2 years of nursing experience, including at least 1 year in home care (geriatrics preferred)
- Strong clinical assessment, organizational, and leadership skills
- Excellent communication and timemanagement abilities
- Dependable transportation for local travel within Suffolk County
- Flexible Per Diem (PRN) scheduling
