What You’ll Do
As the LPN Intake Coordinator, you’ll be the heartbeat of our office operations and intake process. Responsibilities include:
- Answering phones and managing fax communications
- Entering patient referrals into the EMR system
- Assisting with coordination of home health services
- Creating, modifying, and maintaining forms and records
- Filing, distributing Medicare paperwork, and tracking physician orders
- Assembling admission packets
- Supporting quality assurance and regulatory compliance efforts
- Jumping in to help your team whenever needed
- Active LPN license (required)
- Valid driver’s license & reliable transportation
- 1 year of home health experience (required)
- Experience scheduling home health referrals (preferred)
- Proficiency with Microsoft Office Suite (Word, Excel, etc.)
- Ability to participate in on-call rotation
