Full-Time RN Clinical Manager – Hospital at Home Program
Location: Stuart to Vero Beach, FL (Hybrid)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Salary: $72,000 annually Benefits

Key Responsibilities:
Manage patient intake, authorizations, and care planning for skilled and non-skilled services
Conduct start-of-care and supervisory visits, as well as orientations and training for staff
Provide clinical oversight aligned with best practices and agency policies
Support and mentor field clinicians to ensure quality care delivery
Maintain compliance with all regulatory and accreditation requirements
What You’ll Bring:
Active RN license in Florida (required)
Minimum 1 year of home care experience (preferred)
Experience with Hospital at Home programs (highly preferred)
Valid driver’s license and reliable transportation
Strong communication, leadership, and time management skills
EMR proficiency; AlayaCare experience a plus
