Summary: 

As a Community Coach at AccessStar Community Living Services, you will play a pivotal role in supporting individuals with developmental disabilities in the community and within our Beacon Day program. You will provide personalized assistance, guidance, and encouragement to help individuals thrive and participate actively in community life. 

Essential Duties and Responsibilities: 

(All persons in this position must be capable of performing these functions, in adherence to all ASCLS Policies and Procedures) 

  • Encourages the individuals supported to participate in their goals/outcomes, social, educations, and recreational activities. 

  • Ensures that the individuals supported are provided their Human Rights to include but not limited to: personal choices in all aspects of their daily life, and are empowered to pursue their own choices and ambitions 

  • Ensures the safety of the individuals supported by providing supervision of the individuals at all times according to the Individual’s Service Plan and ASCLS Staffing Plan 

  • Implementation of all required supports, guidance, and instructions of an Individual’s Service Plan for all the individuals being supported by ASCLS on a daily basis 

  • Provides input for the development of services for the individuals supported by ASCLS.  This may include but not be limited to: writing, typing, or proof-reading documents as needed. 

  • Completes and maintains all required documentation in logs, electronic health records (Practical Health Systems, etc.) 

  • Establishes and maintains cooperative and professional relationships with ASCLS staff, outside stakeholders, families/guardians of the individual, and the individual supported 

  • Must be able to able to implement Behavioral De-escalation Techniques and strategies to manage crisis situations and various other emergency situations and follow ASCLS process and procedures. 

  • Schedules, implements, and follow-ups on various Community related activities for the individuals supported. 

  • Provides trainings, assistance, guidance, and varying levels of physical supports to the individual supported.  Individuals Supported may need support in, areas of personal hygiene, general wellness, activities of daily living (bathing, household chores, etc.), preparing and learning about nutritionally balanced meals, communication, socialization, appropriate use of leisure time, shopping, money management, use of community resources, as necessary for each individual. 

  • Reports on any issues with the facility, agency vehicles, or adaptive equipment for the program and individuals supported. 

  • Requisitions needed materials such as office supplies, stamps, latex gloves, activity supplies, etc., in accordance with ASCLS established procedures. 

  • Transport in ASCLS vehicles or in own vehicle, as necessary, the individual supported to and from various community events, shopping trips, recreational events, volunteer sites, and scheduled classes. 

  • Implementation and documentation of the Behavioral Support Plans for the individual supported and participates in the development of the plan. 

  • Required to complete all of ASCLS’s training and orientations and must maintain compliance with all trainings. 

  • Attends relevant and scheduled staff meetings. 

  • Performs other duties as assigned. 

Supervisory Responsibilities: 

This position has no supervisory responsibilities of other ASCLS employees 

Qualifications:

Qualifications:

Education and/or Experience: 

High School Diploma or Equivalent Experience Preferred 

Language Skills: 

Ability to speak, read, and interpret documents in English, such as safety rules, operating and maintenance instructions, and procedure and policy manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively with others. 

Mathematical Skills:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. 

Reasoning Ability: 

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited guide rules exist.  Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 

Certifications, Licenses, Registrations: 

The Community Coach is required to maintain CPR/First Aid Certification and to receive required training and certifications in such other areas as are necessary for ASCLS licensure.

While performing the duties of this job, the employee is regularly required to stand, walk, and sit; use hands to finger, handle, or feel; reach with hands and arms; taste or smell; and talk or hear. The employee may occasionally be required to climb or balance, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds, and frequently must provide physical support to individuals weighing 200 pounds or more. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. 

Work Environment: 

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. 

The employee is regularly exposed to outdoor weather conditions. The noise level in the work environment is usually quiet to moderate. 

Salary : $17 - $19
