Collaborate with interdisciplinary teams, educate residents and caregivers, and participate in care planning to promote resident independence and mobility.
Qualifications include a current California PT license, graduation from an accredited program, and strong communication and documentation skills. SNF experience is preferred but not required, with opportunities for new graduates.
