Role & Responsibilities
This position involves providing home health visits to patients, managing a schedule of approximately 5-6 visits daily, with 5-10 patients assigned per day. You will document visits within 24 hours using the HomeCare HomeBase EMR, ensuring accurate and timely records to support patient care and billing. Responsibilities include travel up to 75 miles, with mileage reimbursement, and participation in office meetings. Accepting patients regardless of location is essential to prevent cancellations. Flexibility may be required for weekend shifts.
Qualifications & Skills
Minimum 1 year of adult home health experience, current license, CPR/BLS certifications, reliable transportation, valid driver’s license, and agency-required liability insurance. Must understand role expectations and demonstrate professionalism and organizational skills.
