- Develop and coordinate individualized discharge plans based on team input and patient needs.
- Conduct biopsychosocial assessments through patient and family interviews to identify treatment priorities.
- Manage care for high-acuity patients with complex mental health and substance use disorders.
- Lead therapeutic groups utilizing evidence-based modalities.
- Facilitate family therapy and discharge planning to ensure safe community reintegration.
- Participate in treatment team meetings, contributing social work insights to treatment plans.
- Report patient issues to community or state agencies as mandated.
- Represent the facility in legal hearings related to mental health holds and conservatorships.
- Master's Degree in Social Work.
- Current LCSW or working towards licensure.
- Knowledge of acute mental health diagnoses and legal holds (5150, 5250, 5270).
- Basic Life Support (BLS) certification.
- Experience in an inpatient psychiatric setting preferred.
