Location: 1603 Court Street, Syracuse NY 13208
Pay Rate: $20.00 Hourly
Schedule: M-F: 8 a.m. 4 p.m.

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role:
We are looking for a proactive employee to use ones own experience and judgement to assist families of youth receiving Critical Time Intervention and Transitional Residential Settings services.
Responsibilities include:

- Provide strength-based, family-centered support to families of youth receiving CTI/TRS services; set up consistent, scheduled meetings in the family home or mutually agreed upon location
- Assist the family/caregiver to determine needs that will help stabilize the youth and family unit; assist the family/caregiver in developing goals to work on
- Assist the family/caregiver to find resources for their family to maintain stability
- Participate in all scheduled meetings with the youth/family and team; attend other meetings with family/caregiver, as requested
- Schedule and participate in a closing meeting for youth/family at end of CTI process

- Complete all program documentation in a timely manner, including but is not limited to: service notes, behavioral data, program outcomes, etc.
- Complete incident report forms thoroughly and in a timely manner
- Provide person-centered, quality, services
- Observe confidentiality and safeguards regarding all participant related information
- Perform Other Duties as Assigned
Education & Experience
Lived experience as a family/caregiver of a child/youth with a social, emotional, behavioral, mental health, or developmental disability, required. Family Peer Advocate certification preferred, or must obtain certification within 6 months of hire date.
Conditions of Employment
Must have a valid NYS drivers license in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
