Certified Home Health Aide (CHHA)

Job Overview
We are seeking a compassionate and dedicated Certified Home Health Aide (HHA) to join our team in providing exceptional in-home care to clients with diverse needs. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Monitor and observe patients’ conditions regularly, noting any changes or concerns related to health or behavior.
• Administer medications and treatments as prescribed, ensuring proper documentation in Electronic Health Record (EHR) systems while maintaining HIPAA compliance.
• Perform medical procedures such as catheter care, vital signs measurement, Hoyer lift operation, and basic wound care under supervision.
• Support clients with meal preparation, light cleaning, laundry, and maintaining a safe home environment.
• Document all patient interactions accurately using medical documentation standards and EMR systems.
• Provide specialized care for individuals with dementia, Alzheimer's disease, developmental disabilities, or those receiving hospice or memory care services.
• Use medical terminology proficiently to communicate effectively with healthcare professionals and family members.
• Follow infection control protocols and safety procedures during all caregiving activities.
• Collaborate with nursing staff and other healthcare providers to ensure comprehensive patient care plans are followed.

Qualifications

• Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
• Prior experience working in assisted living facilities, nursing homes, or home health environments preferred.
• Demonstrated ability to perform patient observation and assist with ADLs efficiently.
• Knowledge of medical terminology, medication administration, and vital signs monitoring.
• Experience working with individuals with developmental disabilities or dementia is highly desirable.
• Proficiency in using EMR/EHR systems for documentation purposes.
• CPR certification required; additional certifications such as hospice care or catheter care are a plus.
• Ability to lift patients safely using equipment like Hoyer lifts; physical stamina to support active caregiving tasks.
• Strong communication skills to interact effectively with patients, families, and healthcare teams while maintaining confidentiality per HIPAA regulations.
