Role Description This full-time, on-site Medical Social Worker role is based in Orange, CA. The Medical Social Worker provides psychosocial assessments, supportive counseling, and resource coordination for patients and families in collaboration with the interdisciplinary healthcare team. Daily responsibilities include assessing social, emotional, and environmental needs; developing care plans; and facilitating access to community resources, financial assistance, and support services. The role also includes discharge planning, coordinating safe transitions of care, and advocating for patient rights and preferences. The Medical Social Worker documents all interventions in accordance with regulatory and organizational standards and participates in team meetings, care conferences, and quality improvement activities.

Qualifications
- Demonstrated skills in Medical Social Work and Social Work, including psychosocial assessment, counseling, and patient advocacy.

- Knowledge of Social Services systems, community resources, and eligibility requirements for support programs.
- Bachelor’s or Master’s degree in Social Work or a closely related field; licensed or license-eligible in the state of California preferred.
- Ability to work effectively within an interdisciplinary healthcare team and communicate clearly with patients, families, and providers.
- Strong organizational, documentation, and time-management skills, with the ability to manage multiple cases and priorities.
- Experience in hospital, home health, hospice, or similar healthcare settings is highly beneficial.
