What are the responsibilities and job description for the Physical Therapist - Home Health Flexible hours, Pay per visit position at ACCESS THERAPY LLC?
Access Therapy is in need of driven and dedicated Physical Therapists (PTs) for home health to provide excellent home care solutions to individuals of adult ages. Our PTs are highly-educated and experienced in Geriatric and Home health cases who work with your patients to reduce pain, improve or restore mobility and balance recovering from surgery or hospitalization. The main PT goal is to reach your patient’s prior level of function and/ or safe independence at home.
*We are open to hiring W2 employees, 1099 contractors, per diem, part time, full time*
Qualifications:
- Bilingual a plus, but not a requirement
- Completion of a Physical Therapy curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the American Medical Association.
- Physical Therapy license and registration by the state of agency of practice.

Responsibilities:
- Assessment and evaluation of patient care needs related to getting patients back in motion with exercises, massage, and other techniques, and other physical therapy needs as defined by medical conditions.
- Helps to determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and charts and monitors the patient’s progress.
- Implement and adjust patient treatment plans as needed.
- Communicate with patients, caregivers, and their families regarding the patient's progress.
- Create a clean and safe environment for patients.
Skills:
- Candidate must have knowledge of physical therapy methods, theories and principles in the application, as such, in a rehabilitation setting.
- Must possess profound skills in providing various treatments.
- Communication skills in both written and oral form must be exceptional.
- Must be very good in preparing records, and in maintaining them with accuracy and discretion.
- Has the ability to recognize when certain patient protocols need to be modified, to meet the patients' appropriate needs, and reports it dutifully.
