Pay Rate: $21.00 per hour
Hours: Monday-Friday 8:30AM-5:00PM (40hrs)
Overview Of Primary Responsibilities
Supported Housing is a community rehabilitation and support program to assist individuals in acquiring and maintaining skills and supports to live successfully in their communities. Supports include wellness, self-management, benefits and financial management, leisure planning, and skill development to manage rehabilitation and to practice life skills. This program offers recovery support services to assist individuals in obtaining specific life goals while reducing the risk of relapse, hospitalization, loss of housing, or involvement with the criminal justice system.

Primary Functions
- Conduct job search and development activities to assist clients in obtaining employment
- Provide individualized follow along supports to assist clients in maintaining employment
- Provide benefits counseling, provide education to family members and employers
- Negotiate job accommodations when needed and work collaboratively with team members to achieve employment outcomes
- Perform other duties as assigned
- Participate on agency work-related value streams
- Ability to work as an effective team player and the ability to achieve employment outcomes
- Excellent interpersonal and engagement skills
- Good organizational skills
- Bilingual English and Spanish speaking a plus
- Bachelor's degree in Human Services or related field, or Associates degree in Human Services with 2 years’ experience
- 2 years of experience working in case management or mental health services and/or equivalent experience

- Must be capable to sit or stand in front of a computer for long-periods of time
- Must be able to demonstrate certain tasks that an individual may have to do at the job site
- Able to work in open space floor plan
- Must be capable to move throughout work day (and follow individuals throughout the community)
- Work alongside co-workers within 3 feet
- Occasional lifting of 10 pounds
