Assertive Community Treatment (ACT) Team โ€“ Community-Based Role

Are you passionate about using your lived experience to inspire hope and recovery? Join our Assertive Community Treatment (ACT) team and make a meaningful impact in the lives of individuals living with Serious Mental Illness (SMI).

ACT is an evidence-based, person-centered, recovery-focused model that delivers intensive, mobile behavioral health services in the community. Our interdisciplinary team meets individuals where they are โ€” at home and in the community โ€” to support housing stability, wellness, employment, relationships, and symptom management. We utilize trauma-informed care, cultural competence, and a harm reduction approach to help participants achieve their self-identified recovery goals.

As an ACT Certified Peer Specialist, you will play a vital role by bringing the power of lived experience into treatment planning, crisis support, and daily engagement.

๐Ÿ’› What Youโ€™ll Do

  • Share your lived experience to inspire hope and support recovery
  • Collaborate with participants, families, and interdisciplinary team members
  • Manage a caseload and serve as chart manager for assigned participants
  • Provide skill-building, coaching, and recovery-oriented supports
  • Advocate for participant voice, choice, and self-determination
  • Contribute to treatment planning, crisis intervention, and team decision-making
  • Participate in daily team meetings and care coordination
  • Complete timely and compliant documentation (progress notes, assessments, treatment plans)
  • Participate in on-call rotation
  • Represent participant perspectives with community partners and stakeholders
  • Support quality improvement initiatives through participant feedback

  • Demonstrated lived experience with mental health challenges and willingness to share in a professional, supportive way
  • Strong communication, organizational, and documentation skills
  • Ability to prioritize tasks, manage boundaries, and remain flexible in a fast-paced environment
  • Commitment to teamwork, cultural humility, and trauma-informed practice
  • Motivation to engage in ongoing training and professional development

๐ŸŽ“ Qualifications

  • High School Diploma or GED required
  • NYS Certified Peer Specialist (Provisional accepted)
  • Lived experience with mental health recovery (required)
  • Prior experience in a supportive role preferred
  • Valid NY State Driverโ€™s License required
  • Bilingual (Spanish, Haitian Creole, or Yiddish) a plus

๐Ÿš— Work Environment & Physical Requirements

  • Community-based work โ€” must be able to travel throughout the day
  • Ability to sit or stand for extended periods
  • Comfortable working in open office environments
  • Occasional lifting (e.g., backpack with laptop/materials)
  • Ability to navigate tight spaces and accompany participants in the community
