HIV Patient Navigator

Position Schedule

Please note that ASN operates under an approved Alternative Workweek Schedule. This position is regularly scheduled Monday through Thursday from 8:00 a.m. to 6:00 p.m.

Position Summary

The HIV Patient Navigator provides client-centered case management, education, advocacy, and referral services to individuals living with HIV/AIDS and related health conditions. This position supports clients through health education, care coordination, and community outreach while maintaining confidentiality, professionalism, and culturally responsive services.

The ideal candidate demonstrates strong interpersonal skills, sound judgment, professionalism, and a commitment to serving diverse and underserved populations.

Minimum Qualifications

  • Bachelor’s degree required.
  • Bilingual in English and Spanish is highly preferred
  • Demonstrated knowledge of issues affecting individuals living with HIV/AIDS, Hepatitis C, substance use disorders, and underserved populations.
  • Experience in outreach, case management, public health, social services, or related direct-service work preferred.
  • Ability to engage clients and community members in respectful, nonjudgmental, and trauma-informed discussions regarding HIV, Hepatitis C, sexual health, and related health concerns.
  • Strong verbal and written communication skills in English sufficient to perform essential job duties.
  • Ability to maintain accurate and timely documentation and records.
  • Strong organizational skills with the ability to prioritize tasks and meet deadlines.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Demonstrated professionalism, discretion, and ability to maintain client confidentiality and appropriate professional boundaries.
  • Ability to advocate effectively for clients with healthcare providers, treatment providers, social service agencies, and other community partners.
  • Demonstrated sensitivity and respect for individuals of diverse backgrounds, including race, ethnicity, gender identity, sexual orientation, socioeconomic status, and lived experiences.
  • Proficiency in basic computer and telephone communication systems.
  • Positive, solution-oriented approach to problem solving.

Essential Duties and Responsibilities

  • Conduct intake assessments for new clients and evaluate service needs.
  • Complete ongoing client assessments and assist clients with health enhancement strategies, including nutrition, exercise, stress reduction, and wellness planning.
  • Assess HIV transmission risk behaviors and provide education, risk reduction counseling, and behavior change support.
  • Provide clients with general education regarding HIV/AIDS treatment options, laboratory terminology, medications, and available community resources. (This position does not diagnose medical conditions or provide medical advice beyond scope of training.)
  • Coordinate referrals for healthcare-related services, including dental care, nutrition services, psychosocial support, behavioral health services, and complementary therapies.
  • Advocate for clients with healthcare providers, pharmaceutical assistance programs, and social service agencies; accompany clients to appointments when appropriate.
  • Serve as a liaison among healthcare providers, social service agencies, clinics, and supportive service programs.
  • Provide case management services for eligible AIDS Waiver and related program participants.
  • Assist with family education and support referrals as appropriate.
  • Disseminate current and accurate HIV prevention and treatment information to clients and community stakeholders.
  • Provide patient education and support risk reduction strategies through one-on-one and group interactions.
  • Develop, coordinate, or assist with educational workshops, prevention activities, and community presentations related to public health and HIV prevention.
  • During outreach activities, explain the purpose and services of ASN programs to community members, partner organizations, and other stakeholders as appropriate.

Work Environment and Physical Requirements

  • Position may require local travel, community outreach, and attendance at off-site meetings or events.
  • Ability to communicate effectively in person, by phone, and through electronic communication methods.
  • Ability to perform sedentary and light physical activities associated with office and outreach work.
