The Opportunity:
We are seeking Full Time Case Managers to support our patients at our Delaware office. Case Managers are a vital part of our interdisciplinary team and work to ensure continuity of care, community connection and lasting recovery.
Qualifications:
-Master’s and Bachelor’s degree in social work/psychology OR associates degree in human services/substance abuse preferred. Will consider experienced case managers with demonstrated work performance and no degree.
-Previous experience in Community Based Mental Health, AOD, Addiction and Recovery helpful.
-Previous experience with Medicaid/MITS portal helpful.
Key Responsibilities:
-Provide case management services for a caseload of clients with mental health and/or substance abuse diagnosis.
-Conduct initial needs assessments and develop individualized care plans.
-Assist clients with accessing community resources, including housing, food security and healthcare.
-Collaborate with therapists, physicians and other team members.
-Maintain accurate documentation of services, progress notes and communications in accordance with agency and licensing standards.
-Attend staff meetings and trainings as required.
-Provide education and advocacy to support client stability and self-sufficiency whether in office or out in the community.
Who We’re Looking For:
-Someone with strong communication, organizational and interpersonal skills.
-Knowledge of local social services and behavioral health systems.
-Ability to work independently and as part of a multidisciplinary team.
-Has a valid Driver's license and reliable transportation
-Ability to pass required background check, drug testing and meet any credentialing/licensing requirements applicable for the position.
Schedule & Pay Scale:
-Full Time M-F, 40 Hours per Week.
-Schedule options include: 8:00am-4:30pm, 8:30am-5:00pm or 9am-5:30pm
- HS Diploma/Associates- $19-$21 per hour
-Bachelors/Masters- $20-$23 per hour
Job Type: Full-time
Pay: $19.00 - $23.00 per hour
Expected hours: 40 per week
