Position Summary
The Referrals Coordinator – Internal Referrals is responsible for managing the full internal referral lifecycle within the EPIC system, from order entry through referral closure. This role ensures referrals meet documentation standards, internal clinical guidelines, and service-level expectations while maintaining accurate tracking within designated work queues. The position supports timely patient access, minimizes referral leakage, and promotes compliance with established internal workflows.
Core Job Responsibilities
- Conduct proactive patient outreach to provide education regarding internal referral processes, appointment preparation, and next steps, ensuring patient understanding and engagement.
- Coordinate with internal sites to ensure appointments are scheduled, and patients receive necessary appointment options.
- Coordinate scheduling across internal departments to ensure timely appointment access in alignment with established turnaround time standards and service-level expectations.
- Monitor and reconcile internal referral work queues daily, resolving scheduling barriers, documentation deficiencies, and workflow errors to prevent referral leakage.
- Route referrals accurately to internal specialty departments using established service line mapping and scheduling protocols.
- Apply internal triage guidelines to appropriately prioritize high acuity versus routine referrals, escalating urgent cases according to policy.
- Collaborate with specialty departments, site leadership, and central scheduling teams to optimize referral workflows and reduce delays in patient access.
- Participate in continuous quality improvement initiatives, training activities, and process optimization efforts to enhance internal specialty access and referral efficiency.
- Address open referrals according to policy and procedures.
- Other duties as assigned.
Requirements/Preferences
- High school diploma or GED required
- Minimum 1 year experience in a clinical office or health care environment in patient registration, referrals, medical assisting or customer service.
- 1 year EHR systems experience, preferred, EPIC a plus
- Bilingual/Spanish preferred
- Beginning proficiency Microsoft Office Suite (Word, Excel, Outlook)
Working Conditions/Equipment
- Normal office environment regularly
- Health Center environment occasionally
- Local travel between health centers and community events is required; mode of transportation required
- If personal vehicle is employed, a current driver’s license and proof of insurance is required
ACCESS is a Network of Federally Qualified Health Centers treating patients on the frontlines of community-based health care. Depending on position applied/being recruited for, candidates may be required to be vaccinated against communicable diseases and provide supporting documentation proving that they are properly vaccinated, or apply for religious and/or medical vaccination exemption as a part of the application process.
The pay ranges provided represent the minimum to mid-range for positions. Actual compensation will be determined based on a combination of factors including years of experience, educational background, market conditions, and available grant funding.
Salary : $17 - $20
