As an Integrated Health Manager, you will lead care delivery and clinical operations transformations that improve throughput, utilization, and frontline performance for providers and payers. Your primary responsibilities may include:
- Design and scale preventive, chronic, and complex care programs to close care gaps
- Lead care coordination and transitions‑of‑care initiatives across payer, provider, and community settings
- Support value‑based care strategy, operating model design, and performance management
Travel

Here's what you need:
Minimum 5 years of consulting experience with healthcare payer and/or provider clients
Minimum of 2 years of experience in population health, care management, or value‑based care programs
Bachelor's Degree
Preferred Qualifications:
Experience supporting population health platforms or care management technologies
Exposure to Medicaid, Medicare Advantage, or ACO operating models
Background in health equity, community‑based care, or behavioral health integration
Strong understanding of longitudinal and cross‑sector care models
Compensation at Accenture varies depending on a wide array of factors, which may include but are not limited to the specific office location, role, skill set, and level of experience.
