Responsibilities
- Conduct thorough patient assessments, including physiological evaluations and diagnostic evaluations for diverse populations such as pediatrics, mental health patients, and individuals with disabilities or substance use disorders
- Develop and implement individualized care plans while coordinating discharge planning and post-acute care services to ensure seamless patient transitions
- Manage electronic health records (EHR) accurately in compliance with HIPAA regulations and hospital policies
- Collaborate with multidisciplinary teams to facilitate inpatient, outpatient, and clinic-based care delivery across various specialties including hospital medicine, emergency medicine, and behavioral health
- Provide patient education on medical terminology, treatment options, medication management, and community resource utilization to promote informed decision-making
- Engage in utilization management activities to optimize patient care efficiency while maintaining quality standards
- Support social work functions such as community resource coordination, intake assessments, case management, and working with families to ensure comprehensive support for patients’ needs
Experience
- Proven hospital experience with exposure to Level I Trauma Centers or Level II Trauma Centers is highly preferred
- Extensive acute care experience across inpatient units such as ICU, PICU, or emergency departments is essential
- Demonstrated experience working with adults and individuals with disabilities or mental health conditions in inpatient or outpatient settings
- Familiarity with managed care processes and utilization management practices within hospital or clinic environments
- Proficiency in electronic health records (EHR) systems and medical records management is required
- Strong background in behavioral health assessment, addiction counseling, social work principles, and discharge planning processes
- Knowledge of medical terminology, physiology fundamentals, diagnostic evaluation techniques, and client communication strategies is necessary
- Ability to prioritize tasks efficiently in high-pressure situations while maintaining excellent patient service standards
