What are the responsibilities and job description for the Rehabilitation Physical Therapist position at ACC- Ambulatory Care Center?
The Physical Therapist organizes and conducts medically prescribed physical therapy programs in the hospital to restore function and prevent disability following disease, injury, or loss of body part. Administers and directs administration of physical agents, exercise, and gait training utilized in physical therapy programs, as prescribed by referring physician. Services are provided in outpatient and/or inpatient acute care settings.
Qualifications:
- Bachelor's degree in Physical Therapy from an accredited institution.
- Registered Physical Therapist in the state of Illinois.
- Minimum of one (1) year of experience, however new graduates will be considered.
Responsibility Highlights (may vary based on the setting):
- One on one patient care.
- Provide physical therapy evaluation, treatment and follow up services to physician referred patients.
- Create individualized plan of care and home exercise program to help patients reach their functional goals.
- Utilize CPT codes including therex, neuromuscular re-education, gait training, therapeutic activity, and manual therapy to treat patient.
- Educate use of DME such as wheelchairs, braces, canes, crutches.
- Evaluate functional capabilities and provide safe discharge and/or equipment recommendation.
- Work in a multidisciplinary team of doctors, nurses, social workers to help improve patient outcomes.
Pay: $39.00 - $70.98 per hour
