We are seeking a dedicated Hospice Nurse Practitioner to join our compassionate team. In this role, you will play a crucial part in delivering high-quality hospice care to patients and their families, ensuring comfort and dignity during end-of-life transitions.
Responsibilities
- Provide comprehensive nursing assessments and develop individualized care plans for hospice patients.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Collaborate with interdisciplinary teams to coordinate patient care and support services.
- Educate patients and families on end-of-life issues, symptom management, and available resources.
- Monitor patient conditions and communicate changes to the healthcare team promptly.
- Maintain accurate medical records and documentation in compliance with regulatory standards.
- Participate in on-call rotations as needed to provide timely support for patients.

Qualifications
- Current RN license in the state of Texas.
- Minimum of 2 years nursing experience, preferably in hospice or palliative care settings.
- Strong assessment skills with a focus on patient-centered care.
- Excellent communication and interpersonal skills for effective collaboration with families and healthcare teams.
- Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) preferred.
- Familiarity with EMR systems for accurate documentation.
- Ability to work independently while managing multiple priorities effectively.
- Compassionate approach towards end-of-life care.
