Role Description
This is a full-time, on-site Hospice Social Worker role who would serve communities surrounding Brenham. The primary responsibilities include providing social work services to support patients and their families as they face end-of-life challenges. Tasks involve conducting assessments, creating individualized care plans, coordinating respite stays or permanent facility placement, offering grief counseling, and serving as a resource to guide clients through difficult times.
Qualifications
- Proficiency in Medical Social Work and general Social Work practices
- Experience in End-of-Life Care and Grief Counseling
- Strong skills in resouce utilization and coordination of services
- Compassion, empathy, and excellent interpersonal skills
- Master’s degree in Social Work (MSW) from an accredited institution
- State licensure or certification as required for the role
- Previous experience working in hospice or palliative care settings is a plus
