Role Description This position of Hospice Nurse is a PRN role based out of our office in Montgomery, TX. This role provides direct patient care, including ongoing assessment, symptom management, medication administration, and coordination of palliative treatments in homes and facilities. The Hospice Nurse collaborates with physicians, interdisciplinary team members, patients, and families to develop and implement individualized care plans. Daily responsibilities include educating patients and caregivers about disease progression and comfort measures, documenting care accurately, and participating in care conferences and on-call rotations. The role requires consistent, compassionate support to patients and families, ensuring their physical, emotional, and psychosocial needs are met.
Qualifications.
- Expertise in Palliative Care, including pain and symptom management and end-of-life care planning preferred
- Experience in Home Care and Direct Patient Care, providing compassionate, hands-on support in patient residences and facilities preferred
- Current, unrestricted Registered Nurse (RN) license in the state of Texas required
- Previous hospice or home health experience preferred
- Excellent communication, interpersonal, and documentation skills
- Ability to work collaboratively within an interdisciplinary team and independently in the field
- Valid driver’s license, reliable transportation, and ability to travel locally to patient locations
