Role Description
This is a full-time RN Case Manager role based out of our Montgomery, TX office. The Hospice Nurse will provide compassionate and individualized care for patients experiencing life-limiting illnesses. Responsibilities include administering medical treatments, offering guidance on a palliative to care, and delivering direct nursing care to ensure patient comfort and dignity. The nurse will work closely with families, caregivers, facility staff members, and interdisciplinary teams to align care plans with the patient’s and family’s objectives and preferences.
Qualifications
- Clinical skills in Nursing and Direct Patient Care with a compassionate approach to patient support.
- Experience in Home Care, including the ability to provide care and guidance to patients and their families wherever they call home (private residences, assisted living, skilled nursing facilities, personal care homes).
- Excellent communication and interpersonal skills to collaborate effectively with patients, families, and healthcare team members.
- Current and valid Registered Nurse (RN) license in the state of Texas.
- Previous experience in hospice or palliative care is a plus.
- Ability to demonstrate empathy, patience, and understanding in end-of-life scenarios.
