Essential Job Functions/Responsibilities:
- Completes an initial comprehensive assessment of patients.
- Initiates the plan of care.
- Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare) prior to end of shift.
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
- Teaches the patient and family/caregiver selfcare techniques as appropriate.
Service Area Coverage: Orange County
Schedule:
- Parttime, Per Diem
- Shift: Weekends, Holidays
- Weekdays after business hours patient admissions as needed
Position Qualifications
Experience:
- Hospice: 1 year (Required)
- Admission Nurse Experience (Required)
- HomecareHomebase (Pointcare) Experience Preferred
License/Certification:
- CA RN License
- CPR certification
- CA Driver License
Location: Field-Based Position. This role involves traveling to provide care in various settings, including:
- Patients’ homes
- Skilled nursing facilities (SNFs)
- Boardandcare homes
