Essential Functions
|

Responsible for coordinating the completion of the MDS and ensures the resident’s care plan is developed in accordance with State and Federal guidelines. |
|
|
* |
Maintains current and accurate information regarding the resident’s status and need for assessment: a.) On admission b.) When there is a significant change in condition c.) For annual review d.) For quarterly review And maintain schedule/calendar to ensure these targets are fulfilled. |
|
|
* |
Assigns specific sections of the MDS to the appropriate department for completion and ensures all sections are completed according to the scheduled dates. |
|
|
* |
Provides inservice training as needed to all departments regarding the accurate completion of the MDS and changes in the MDS/RUGS Process. |
|
|
* |
Assign assessment data based on the established calendar and insures all sections are completed accordingly. |
|
|
* |
Signs in the MDS booklet to certify completion. |
|
|
* |
Schedules the Comprehensive Care Plan committee meeting and assists as needed in developing the resident’s care plan. |
|
|
* |
Attends Comprehensive Care Plan meetings providing the care plan team with guidance on efficiency and requirements for care planning. |
|
|
* |
Coordinates timely completion and transmission of the MDS with the MDS secretary. |
|
|
* |
Conducts Quality Assurance audits as assigned and submits evaluation of these results in a timely manner. |
|
|
* |
Communicates effectively on all levels, with all departments both orally and in writing. |
|
|
* |
Insures all code and regulatory requirements are met. |
|
|
* |
Maintains a professional appearance and attitude. |
|
|
* |
Provides assistance with resident care when applicable. |
|
|
* |
Adheres to all facility and departmental policies and procedures |
|
|
* |
Follows facility Fire and Safety procedures and participates in ‘mock’ drills. |
|
|
* |
Reports any potentially dangerous situations or accidents/incidents immediately. |
|
|
* |
Maintains confidentiality of organizational and resident information. |
|
|
* |
Informs the Administrator and/or the Director of Nursing of any potentially problematic reimbursement issues regarding resident coverage. |
|
|
* |
Notifies resident families or responsible parties of changes in resident insurance coverage both verbally and in writing and keeps a log of these notifications. |
|
|
* |
Schedules and maintains ongoing Medicare meetings to review coverage and continued stay/discharge plans with all departments. |
|
|
* |
Assists in checking the accuracy of the ‘802’ form information and preparing it for the Survey team in a timely manner. |
|
|
* |
Serves as the Facility Electronic Medical Record Coordinator insuring the system integrity, assignment and removal of access, trouble shooting nursing and system related issues. |
|
|
* |
Serves as primary contact for the facility related to Corporate Electronic Medical Record initiatives. Chairs the facility Electronic Medical Record Committee and participates in Corporate related meetings and discussions related to the Electronic Medical Record. |
|
|
* |
Has overall responsibility for the facility Employee Health Program Ensures that all employees receive and are offered; Annual and New employee health assessments, tuberculosis testing, hepatitis B vaccinations, MMRs, Influenza, and pneumonia vaccinations. |
|
|
* |
Tracks and monitors resident vaccinations to ensure they are provided and offered in a timely fashion. |
|
|
* |
Other duties as assigned. |
|
|
* |
Oversees the Medicare Part A and Medicare Managed Care utilization. Review weekly meetings and Medicare Part A/Managed Care Discharge meetings. Works with the Director of Social Services, Case Manager, and DNS on this process. |
|
|
* |
Communicate with Managed Care companies. Coordinate service, coverage approvals and authorizations. |
|
Education & Experience
|
|
Formal Education: Associate Degree or certification equivalent |
|
Experience: 6 months |
|
License, Registration, and / or Certification Requirement: Yes |
|
Minimum Knowledge: Requires extensive knowledge in several specialized disciplines and ability to integrate critical information from many diverse areas. Requires extensive theoretical and practical knowledge. |
|
Language Ability: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to write speeches and articles for publication that conform to prescribed style and format. Ability to effectively present information to top management, public groups, and / or boards of directors. |
