The LPN Assisted Living is a key player in our Resident Care Team. They assists our residents by administering medication and performing routine nursing care and related duties as required.
Responsibilities:
- Administers medications to assigned residents according to professional standards, facility policy, and physician’s orders. Documents response to medication and medication effectiveness as required.
- Ensures the medication cart is cleaned and stocked daily with paper supplies.
- Ensures medication orders are picked up and medication is ordered from the pharmacy and delivered in a timely manner.
- Performs nursing procedures/treatments to assigned residents according to professional standards, facility policy and physicians orders. Documents response and effectiveness of treatments. Includes vital sign, specimen, and weight collections.
- Communicates accurate, up-to-date information to Resident Care Assistants and Memory Care Partners. Provides direction, feedback, and assistance to RCAs and MCPs as needed.
- Produces timely accurate and legally compliant documentation of residents’ status, changes, and observations in resident chart and communication book.
- Educates resident/family members regarding plan of care, medications, procedures and treatments, and participates in Family Meetings as necessary.
- Provides continual monitoring of resident’s physical, social, and mental status. Promptly reports changes and concerns to LPN Manager.
- Maintains and develops work-related knowledge and skills through continuous learning activities, attendance at in-services and workshops, etc.
- Completes special projects/tasks in support of effective department functioning as assigned by supervisor.
Qualifications:
Experience
- 2 years professional nursing experience
- previous experience working with senior population
- experience in Assisted Living is preferred
Education
- High School Diploma/GED
- Current NYS LPN licensure
- Current CPR certification
