What are the responsibilities and job description for the School Nurse - Registered Nurse / Licensed Practical Nurse position at Abundant Life Home Health Agency?
Abundant Life Home Health Agency is seeking nursing staff to join our home health team for pediatric care to attend school and work in the home with the patient.
Job Summary: Provide nursing care to patient in school and home setting. Observe and assess the client and/or caregiver to enhance the quality of life. Demonstrate individualized creativity in educating the patient and/or caregiver. Follow nursing policy and procedure per agency standards. Follow the plan of care according to physician orders. Contribute to the plan of care. Demonstrate understanding of Medicare and Medicaid regulations.
Current cases available:
MT (Clearwater) 5yo male; Gtube; colostomy; Ambulatory (fall precaution); Verbal; DX: Spina bifida.
Open shifts: Wed-Fri 7a-7p
AH (Dade City) 6 yo male, limited verbal, GT feeds, may eat PO for pleasure, ambulatory, vision/hearing loss, Dysphagia. DX: Alport syndrome.. 1 large dog.
Open shifts: Wednesday; Saturday; Sunday 7a-7p
AA (New Port Richey) 7 yo male, type 1 diabetic, cognitive function at level for age, ambulatory, verbal- requires monitoring of Omnipod/Dexcom for uncontrolled D1. Shifts are on school days only.
Open Shifts: Monday-Friday 7:45am-3:00pm
