JOB SUMMARY:
The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.
- Travel to multiple locations to provide nursing and condition specific education to your patients in their homes in a time efficient manner.
- Provide admission, case management, and follow-up visits for patients.
- Administer on-going care for each patient, provide necessary follow-up as directed by the DON.
- Confer with physician in developing the initial plan of treatment based on physician’s orders and initial patient assessment.
- Render hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under DON's supervision.
- Report patient care/condition/progress to patient's physician and DON on a continuous basis.
- Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
POSITION QUALIFICATIONS:
- New graduates are welcome to apply!
- Graduate of an accredited school of nursing.
- Current licensure in Illinois and CPR certification.
- Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.
- Computer skills adequate for data entry of clinical documentation.
- Current Illinois Driver’s License and proof of current auto insurance.
LOCATIONS:
Elgin, South Elgin, and adjacent communities towards Bartlett.
Job Types: Full-time, Part-time
Pay: $55.00 - $120.00 per hour
