Job Overview
We are seeking compassionate and skilled Practical Nurses to provide high-quality home care services to patients in need of medical support within their community. The ideal candidate will have experience in diverse clinical settings, including long-term care, hospice, acute care, and working with special populations such as individuals with developmental disabilities or memory care needs. This role involves delivering personalized patient care, performing assessments, administering medications, and supporting patients' health and well-being in a home environment. Familiarity with electronic health records (EHR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is preferred to ensure accurate documentation and seamless communication within healthcare teams.
Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations.
- Administer medications and injections safely following aseptic techniques and HIPAA regulations.
- Perform IV infusions, catheter care, tube feeding, wound suturing, ostomy care, and ventilator management as required.
- Manage airway and respiratory support equipment; assist with ventilator care and respiratory therapy.
- Document all patient interactions accurately using EMR/EHR systems; maintain detailed medical records in compliance with healthcare standards.
- Coordinate discharge planning and collaborate with interdisciplinary teams for case management and post-acute care planning.
- Provide education on health coaching, medication administration, nutrition, infection control, and chronic disease management.
- Support patients with complex needs such as dementia/dementia care, Alzheimer's care, trauma medicine, or pediatric/neonatal nursing when applicable.
- Ensure infection control practices are followed rigorously to maintain a sterile environment during procedures like phlebotomy or specimen collection.
- Assist with diagnostic evaluations such as laboratory testing or sonography when necessary.
- Maintain proficiency in various EMR/EHR systems including Athenahealth and eClinicalWorks to facilitate efficient patient documentation.
Qualifications
- Valid practical nursing license with current state certification.
- Proven experience in home health nursing or related fields such as hospital medicine, ICU, emergency room, or outpatient clinics.
- Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
- Experience administering injections, IV insertion/infusion, catheterization (including Foley and suprapubic), and ostomy or wound care.
- Familiarity with critical care environments including Level I/II trauma centers or neonatal/pediatric intensive care units (PICU).
- Ability to work independently in a home setting while managing multiple patients' needs effectively.
- Strong understanding of infection control protocols and aseptic technique.
- Working knowledge of healthcare software systems such as Epic or Cerner is highly desirable.
- Excellent communication skills for working with patients of all ages—including children—and collaborating with families and healthcare teams.
