Summary:
The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education and experience. The nurse builds from the resources of the community to plan and direct services to meet the needs of the individuals and families within their homes and community.
Responsibilities:
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Completes regular re-evaluation of patient needs.
- Initiates that Plan of care and make necessary revisions as needed.
- Develops care plans, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
- Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as ordered by the medical provider.
- Counsels and provides education to the patient and family related to the patient's care needs.
- Case Management
- Prepares and updates clinical notes
- Communicates with the Medical Provider regarding the patient's needs and reports any changes in the patient's health. Review physician's orders as necessary and facilitate changes in care plan.
- Provides discharge planning as part of the care plan.
- Participates in an-call duties as defined by company policy
Position Qualifications:
- Graduate of an approved nursing program/school
- Current State of MI Licensed RN with one to two years of recent acute care experience.
- Must possess a valid driver's license and acceptable driving record
