Your Impact
In this role, you’ll play a vital part in ensuring comfort, managing symptoms, and supporting both individuals and their loved ones. Your presence will bring reassurance, clarity, and stability during critical moments.
What You’ll Do
- Build meaningful, supportive relationships with patients, families, and care teams
- Perform initial nursing assessments and ongoing evaluations
- Develop, implement, and adjust individualized care plans
- Deliver skilled nursing care and treatments as ordered by the physician
- Educate patients and caregivers with clarity, compassion, and patience
- Collaborate with physicians, social workers, and interdisciplinary teams
- Maintain timely, accurate documentation and reports
- Contribute to quality improvement initiatives and uphold high standards of care
- At least 2 years of nursing experience, including hospice care
- Strong clinical judgment and assessment skills
- The ability to work independently and problem-solve with confidence
- A compassionate, patient-centered approach to care and education
- Excellent communication, organization, and time management skills
- Physical ability to assist with patient mobility as needed
- Associate Degree in Nursing (ASN)
- Active Ohio RN License
- Valid driver’s license with a clean driving record
