Responsibilities
- Conduct comprehensive evaluations and assessments, and develop individualized treatment plans for patients with a wide range of conditions, including orthopedic injuries, neurological disorders, and post-surgical rehabilitation.
- Ability to work independently and as part of a team, demonstrating adaptability and flexibility in a multi-site environment.
- Implement therapeutic exercises, manual therapy techniques, modalities, and other evidence-based interventions to improve patient mobility, strength, function, and reduce pain.
- Educate patients and their families on proper exercise techniques, home exercise programs, and adaptive equipment use.
- Maintain accurate and timely documentation of patient evaluations, progress notes, and discharge summaries, adhering to all regulatory guidelines.
- Collaborate with physicians, nurses, occupational therapists, and other members of the interdisciplinary team to ensure coordinated and comprehensive patient care.
- Communicate effectively with patients, families, and colleagues, fostering a positive and supportive environment.
- Graduate of an accredited Physical Therapy program (DPT or Master's degree).
- Current Physical Therapist license in New York State
- Proficiency in Net Heath Software preferred
- Experience in skilled nursing facilities preferred
