Responsibilities
Conduct admissions assessments and develop individualized care plans in collaboration with the care team
Coordinate with healthcare professionals and community resources to ensure a comprehensive care approach
Provide education and support to residents and their families, promoting independence and empowerment
Collaborate with the interdisciplinary team to identify and address potential barriers to care
Maintain accurate and up-to-date records and documentation
Participate in quality improvement initiatives to enhance the care experience
RN Admissions Nurse will also be responsible for other duties as assigned by the DON or deisgnee
Requirements
RN Admissions Nurse with a valid RN license in the state of New York
Experience in geriatric care
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Compassionate and empathetic approach to patient care
