Responsibilities:
Conduct thorough assessments of new residents, including medical and social histories, to develop individualized care plans
Collaborate with interdisciplinary teams to ensure coordinated care and communicate effectively with families and caregivers
Develop and implement effective admission and transfer protocols to ensure a smooth transition for residents
Provide education and support to residents and their families on medication management, disease prevention, and overall wellness
Participate in Quality Improvement initiatives to ensure high-quality care and patient satisfaction
Maintain accurate and up-to-date records and reports
Demonstrate a commitment to person-centered care and a passion for working with older adults
Other duties as assigned by DON or designee
