Hours 11am-7pm; 12pm-8pm

About the Role:
RN Admissions Nurse is responsible for conducting thorough assessments of new residents, gathering medical history, and developing personalized care plans in collaboration with our healthcare team.
You will coordinate with our interdisciplinary team to ensure timely and effective care, communicate with residents' families, and provide education on our services and programs.
Your attention to detail, strong clinical skills, and excellent communication abilities will enable you to navigate complex health challenges and build trust with our residents and their families.
Responsibilities:
Conduct initial assessments, evaluations, and documentation for new residents, including identifying needs and developing individualized care plans.
Collaborate with the healthcare team, including physicians, therapists, and other caregivers to ensure comprehensive care.
Communicate effectively with residents, families, and healthcare professionals to ensure a safe and supportive environment.
Monitor and report changes in residents' conditions, documenting assessments and interventions as required.
Participate in quality improvement initiatives to enhance the overall care experience.
Other duties as assigned by DON or designee

Requirements:
RN Admissions Nurse must be a licensed Registered Nurse (RN) with a current license in the state of New York.
