Registered Nurse - Home Health

About the Position

We are looking for a dedicated and compassionate Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Minnesota. This part-time role combines in-home patient visits with remote documentation, care coordination, and follow-up activities, offering flexibility and meaningful one-on-one patient care.

Responsibilities

• Conduct in-home patient assessments and nursing visits
• Develop, implement, and monitor individualized care plans
• Administer medications and treatments as ordered by physicians
• Provide patient and caregiver education
• Monitor patient progress and report changes in condition
• Coordinate care with physicians, therapists, and other healthcare providers
• Complete clinical documentation and charting remotely
• Participate in virtual care planning and team meetings

Qualifications

• Current, active Minnesota Registered Nurse (RN) license
• Home health, hospice, case management, or clinical nursing experience preferred
• Strong assessment and critical-thinking skills
• Excellent communication and organizational abilities
• Proficiency with electronic health records (EHRs)
• Reliable transportation and valid driver's license for home visits
