As a Case Manager, you will:
- Participate as a member of a multidisciplinary team providing holistic, recovery-oriented services.
- Assist in developing and reviewing recovery plans in collaboration with participants, clinical staff, medical providers, and other stakeholders.
- Develop and maintain relationships with community agencies and service providers.
- Connect program participants with critical community-based resources and supports.
- Assist individuals in achieving recovery plan goals through advocacy, education, and service coordination.
- Work directly in the community within the program's service area.
- Transport program participants to appointments and community resources as needed.
- Complete assessments, documentation, and reports in accordance with organizational, funding, licensing, and accreditation requirements.
- Perform additional duties as assigned.

QualificationsRequired
- Two years of experience providing support to individuals involved in behavioral health, addiction recovery, intellectual and developmental disability services, homelessness services, or related human services programs.
- Strong interpersonal and communication skills.
- Intermediate computer skills.
- Ability to work independently in community-based settings.
- Ability to complete documentation accurately and timely.
- Subject to applicable background clearances and employment screenings.
- Bachelor's Degree in Human Services or a related field.
- One year of Case Management experience.
- Experience working within behavioral health or community support programs.

- Flexible schedule based on participant needs and program requirements
- Evening and weekend availability may be required
- Local travel throughout the service area required
- Ability to stand, sit, walk, climb stairs, reach, bend, kneel, stoop, crouch, and perform repetitive motions.
- Ability to lift up to 50 pounds occasionally.
- Ability to safely transport participants as needed.
- Ability to perform essential job functions with or without reasonable accommodation.
