Abode Services, one of the largest and effective nonprofits working to end homelessness in the Bay Area, is seeking a skilled Nurse Case Manager to help support our Alameda County Programs.
About the role: The Nurse Case Manager will work closely with the Psychiatrist and Psychiatric Nurse Practitioner to support participants in making it to appointments, understanding their medications, dealing with pharmacy and insurance issues, and adhering to doctor’s directives. The Registered Nurse Case Manager will complete assessments and treatment plans, conduct home visits and work with participants in the community and provide case management and rehabilitative services.

How You Make an Impact:
- Participate in treatment planning and implementation of services to provide support for participant medication adherence.
- Administer medications and injections. Work with program psychiatrists/psychiatric Nurse Practitioner, pharmacies, insurance companies, and primary health care providers to ensure continuity of care.
- Assess nutritional needs and assist participants with improving their general health.
- Provide clinical case management services, including rehabilitative, brokerage, and collateral services to participants in the following settings: home, community, and office utilizing an Assertive Community Treatment model.
- Maintain the medication room. Work with Behavioral Health Care staff on site certifications and maintaining ongoing compliance.
- Complete assessments and develop treatments plans, on an on-going basis for participants enrolled in the program. Collaboratively work with participants to develop and work on their recovery goals.
- Perform mental status exams, as well as safety assessments that include suicide, homicide, and gravely disabled assessments with participants on an ongoing basis.
- Facilitate psycho-educational and support groups.
- Attend and participate in clinical, administrative, and case conferencing meetings.
- Enter all data on time and correctly to support program evaluation and outcomes tracking.
- Ensure proper and timely documentation of services including written case notes using the BIRP formats and billing services to Medi-cal.
- Collaborate on participant care with Psychiatric NP/PA.
- Assist the Program Manager, Clinical Supervisor, and other staff as needed.
- Other duties as assigned.
How You Meet the Qualifications:
- Associate's or Bachelor’s Degree in Nursing and current California Registered Nurse (RN) license required.
- Drive for appointments and visits for field and home visits.
- Use of personal vehicle and proof of valid California Driver's License and current auto insurance, along with a clean DMV record, is required.
- Ability to work flexible hours, including some weekends and evenings.
PHYSICAL REQUIREMENTS:
- Communicating with others to exchange information; seeing to read a variety of materials.
- Manual dexterity for use of keyboard, tools, controls; repetitive motion that may include the wrists, hands, and/or fingers.
- Remaining in a stationary position, often standing, or sitting for prolonged periods, while at a desk or working on a computer.
- Ability to drive and sit in a car for prolonged periods of time.
- Ability to move between floors, ascending and descending stairs.
- Light work that may include moving or lifting objects up to 25 pounds.
- Ability to squat, bend at the waist, crouch, reach overhead and horizontally, and kneel.
WORK CONDITIONS / ENVIRONMENT:
- Must be able to work in a shared office environment with moderate to high noise level with frequent contact and interruption.
- Multi-level buildings with stairs and/or ramps.
- Frequent travel by car throughout the county region and surrounding areas.
- Work in program service environments, which may include entering program offices, non-agency offices and meeting areas.
