Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility
- Monitor patients’ health by observing vital signs, medication adherence, and overall condition
- Support residents with meal preparation, feeding, and nutritional needs
- Utilize patient transfer methods including Hoyer lifts and manual handling techniques to ensure safe mobility
- Maintain accurate care plans, document patient observations, and communicate effectively with healthcare teams
- Provide companionship and emotional support to promote mental well-being
- Assist with medication administration following HIPAA guidelines and safety protocols
- Support residents during medical appointments or community outings using personal transportation assistance
- Follow infection control procedures by cleaning and sanitizing client areas regularly
- Recognize signs of dementia, Alzheimer’s, autism spectrum disorder, or other neurodevelopmental conditions to tailor care approaches accordingly
Qualifications
- Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments preferred
- Knowledge of patient observation techniques, vital signs monitoring, and medication administration
- Experience with dementia care, memory care, Alzheimer’s care, or hospice care is highly desirable
- Ability to assist with ADLs using proper manual handling and transfer methods such as Hoyer lifts
- Strong communication skills for effective client interaction and documentation
- Certification in CPR and First Aid required; additional certifications in HIPAA compliance or social work are a plus
- Familiarity with patient personal care assistance standards and safety protocols in long-term or community care settings
- Ability to lift heavy objects safely and perform physical tasks involving heavy lifting when necessary
- Experience working with individuals with disabilities or developmental delays is advantageous
- Demonstrated ability to work independently while collaborating as part of a healthcare team
