What are the responsibilities and job description for the Program Facilitator position at ABLE LIFE CARE SERVICES?
We are seeking a dependable and organized Home and Community-Based Services Facilitator to help coordinate services for clients receiving care in their homes and communities. This position assists with scheduling, client communication, staff support, documentation review, incident reporting, and ensuring services are delivered according to the client’s plan of care and Louisiana HCBS requirements.
The ideal candidate is professional, compassionate, detail-oriented, and able to communicate well with clients, families, direct care staff, and support coordinators. Experience with HCBS, PCA services, Medicaid waiver programs, LaSRS/EVV, or home care is preferred.
Requirements: High school diploma , CNA or Social Worker preferred, 5 years experience in home and community based services a must. reliable transportation, ability to pass background checks, strong communication skills, and willingness to complete required training.
