What are the responsibilities and job description for the Case Manager position at Able Case Management Services, Inc?
Responsibilities:
- Provide case management services to individuals with developmental disabilities and autism.
- One year experience in the WV IDDW program.
- Coordinate between agencies to develop individualized service plans.
- Coordinate and monitor services provided by various agencies and organizations
- Advocate for clients and connect them with community resources
- Assist clients in accessing housing assistance programs, such as Section 8
- Collaborate with DRS to support clients in finding employment opportunities
- Mentoring and coordinating service between various agencies to include residential providers, WV BMS, WV DoHS, and guardians.
- Monitor agency progress, ensuring compliance with program requirements
Requirements:
- Bachelor's degree in social work or a related field.
- Experience working with diverse populations, including adult protective services and Medicare.
- Knowledge of local community resources and services
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational skills and attention to detail
- Valid driver's license and reliable transportation.
